• Click ‘Get a Quote’
• Provide the following information
  • Policyholder Name (this is your name or your business)
  • Entity Type
  • Email Address (you will use this to log in, moving forward)
  • Create a Password (to login at InsureMyEquipment.com)
  • Contact First Name
  • Contact Last Name
  • Date of Birth
  • Mailing Address
  • Secondary Email Address (if applicable)
  • Valid Phone Number
• The ‘Additional Verification’ Questions are for our information only, and will not hinder you from getting covered if you do not have an answer to one or more of the questions
  • Name of Current Projects
    • If none, write N/A
  • Number of Years in the Entertainment Industry
    • If none, write 0
  • Website
    • personal or business website is OK. If none, write N/A
  • Previous Insurance Carriers
    • Include insurance companies you have worked with for production equipment only. If none, wrote N/A
• Review the Terms and Agree
• Complete the Insurance Application until you get to the quote
• Once you land on the quote, click ‘Save Quote’
• Now you are able to return at any time to review your quote, apply for a new quote or purchase your policy and issue COIs
